Not every ortho-restorative case is straightforward. Often, orthodontics is used simply to align teeth for a more aesthetic restorative outcome. But sometimes, function—not aesthetics—drives the plan.

In this case, the patient had undergone multiple restorations over her lifetime with different dentists. Each time, her occlusion was set at MIP, which transferred contact inconsistently across the mouth. Her primary complaint was jaw pain. She could not wear an appliance comfortably, and muscle fatigue was constant. Her teeth were also worn, creating functional challenges in addition to minor aesthetic concerns.

In this clinical tip, I walk through a complex ortho-restorative case where the patient’s primary concern wasn’t her smile—it was jaw pain and a lack of a comfortable bite.
We used a combination of deprogramming, mild aligner therapy, and selective restorative work to find her repeatable, comfortable bite and restore balance without over-treating teeth unnecessarily. This approach allowed us to expand the arch, improve occlusion, refine restorations, and create more tongue space—all while minimizing the amount of restorative work required.
