Case Continuum Checklist: Submitting a Case

Are you interested in submitting a case for review during a monthly Case Continuum discussion?


The monthly case discussions are designed to deliver meaningful, high-level clinical insight. These sessions are not about showcasing perfect outcomes. They are about real diagnosis, real decision-making and real learning.


Our goal is to create discussions that allow members to analyze complex cases, debate treatment options, develop stronger diagnostic skills and improve long-term planning. Depth matters more than applause. Substance matters more than polish.


To dedicate a full hour to your case, submissions must include comprehensive, diagnostic-quality information. This is how we elevate the conversation and maximize value for all attendees.

Below is the checklist for submitting a case:

1. Diagnostic Photo Standards

High-quality photos are the starting point for meaningful case conversations. To make sure your case gets the attention it deserves, all required images in the provided Keynote or PowerPoint template must be included.


Photos must be:

  • Properly exposed and color accurate

  • In sharp focus

  • Appropriately retracted

  • Taken with correct head positioning

  • Full resolution (not compressed screenshots)


Why Head Position Matters

Correct head position allows us to truly understand what’s happening clinically. When it’s off, it becomes difficult to properly evaluate:

  • Smile design

  • Incisal plane

  • Midline

  • Cant

  • Facial symmetry

  • Lip dynamics


If the full-face photo isn’t diagnostic, we won’t be able to select the case for discussion. This isn’t about perfection, it’s about giving your case the best opportunity to shine.


2. Required Diagnostic Records

You do not need to have final treatment completed, but you do need enough information to allow true analysis and planning.


Clinical Photos

  • Full face at rest

  • Full face smile

  • Full face profile

  • Full face retracted

  • Full face 12 o'clock

  • Right and left retracted

  • Maxillary occlusal

  • Mandibular occlusal

  • Close-up smile

  • Close-up anterior retracted



Radiographs

  • FMX or appropriate periapicals

  • Bitewings

  • Pan (if relevant)

  • CBCT slices (if indicated and adds value)


Digital Records

  • Intraoral scans or STL files (screenshots acceptable)

  • Articulated models (if available)

  • Occlusal analysis (Occlusogram or equivalent)

  • Digital smile design overlays (if used)


Functional Data (When Applicable)

  • CR records

  • Facebow or digital equivalent

  • Mounted models

  • Wear patterns

  • Joint findings

  • Airway considerations


Please include functional records when they add value to your case and help guide the discussion.


3. Written Case Summary

Every submission must include a written summary with:

  • Chief complaint (in the patient’s words)

  • Medical considerations

  • Periodontal status

  • Structural diagnosis

  • Functional diagnosis

  • Esthetic diagnosis

  • Risk assessment

  • Proposed treatment options (even if undecided)

  • What you are struggling with and what feedback you want


Some of the most powerful conversations come from uncertainty and honest reflection. You don’t need to have all the answers. Your questions are what make this community strong.


4. What Will Not Be Selected (So you know what to expect)

To be respectful of everyone’s time, cases will not be selected if:

  • Photos are incomplete or improperly positioned

  • Only “before and after” images are submitted

  • There is no diagnostic foundation

  • There is insufficient information for an hour of discussion


5. What Makes a Case Exceptional

The cases that tend to create the most meaningful dialogue often include:

  • A clear diagnostic dilemma

  • Multiple viable treatment pathways

  • Functional and esthetic overlap

  • Occlusal complexity

  • Risk-based decision making

  • Long-term perspective

  • Honest, thoughtful questions


This Case Continuum is built on clinical thinking, shared openly and respectfully. When we dedicate an hour to your case, our goal is for it to provide enough depth for attendees to analyze, debate, plan and learn together.
