When creating provisional restorations, color can often appear inconsistent due to variations in the substrate shade and the thickness of the provisional material. I’ll walk you through how I managed both the provisionals and the final restorations to achieve even, natural-looking color — all without making a new provisional implant abutment by utilizing the patient’s existing one.

Material selection is a critical factor in any case, but it becomes even more important when retreating teeth that were aggressively prepped. While beautiful final aesthetics are always the goal, functional demands and the patient’s clinical history must also inform our decisions. In this case, lithium disilicate was chosen and used in a monolithic form, milled and stained without cutback or layering, yet still delivered an esthetic result. In cases like this, ingot selection plays a pivotal role in achieving success.

To address the esthetics, the treatment plan involved deprogramming to alter the vertical dimension and performing veneers and crown-lays on teeth #3-14. Despite no functional complaints, deprogramming was needed to establish the correct vertical using a centric relation position. A restorative approach was chosen to meet the patient’s esthetic goals.​

​In this complex case, the patient has implants, veneers, and crowns, with issues of gray discoloration and slight lip imbalance from previous surgeries. Dental Facial Analysis (DFA) revealed excessive tooth exposure. The plan is to shorten the anterior teeth, possibly use Invisalign to intrude the canines, and select suitable materials. Splinting the implants with pink porcelain to achieve a natural papilla effect is recommended. Effective lab communication is crucial for optimal material selection and esthetic outcomes.​
