The next live case presentation is Tuesday, October 22nd at 8pm EST.
Managing tooth agenesis presents a unique challenge, as we aim to provide both functional support during the growth and development phase and enhance patients' confidence with a beautiful smile. This case discussion will focus on interdisciplinary care involving orthodontics, transitional composite bonding, and the progression toward final implant placement and porcelain restorations.

Achieving Longer Teeth Through Comprehensive Dental Restoration

This case focuses on a patient's request for longer teeth, a common esthetic concern. Though the request seemed simple, an examination revealed underlying issues like wear patterns and bite problems. A treatment plan was developed, including orthodontics to correct alignment before restorative work.

Prioritizing Pre-Restorative Orthodontics for Functional Excellence

The main challenge in this case was the patient's pain, discomfort, and airway issues. Treatment involved finding a comfortable orthopedic position and determining how restorations could restore function while aligning with it. The feasibility of using Invisalign and the extent of restorations were key factors in the planning process.

Minimally Invasive Veneer Case

The patient’s main concern was not showing teeth when smiling, though no functional issues were reported. To address the esthetics, the treatment plan involved deprogramming to alter the vertical dimension and performing veneers and crown-lays on teeth #3-14. Despite no functional complaints, deprogramming was needed to establish the correct vertical using a centric relation position. A restorative approach was chosen to meet the patient’s esthetic goals.

Comprehensive Consultation for Maxillary Restorations and Smile Transformation

The patient sought a restorative consult due to dissatisfaction with previous maxillary restorations, including redness and bleeding. She desired whiter teeth and had concerns about redoing recent crowns. Her dental history included orthodontics, occasional clenching, and difficulty eating certain foods. With her main concern being esthetic, a comprehensive evaluation and potential restorations were needed.

Managing Spacing Across the Entire Upper Arch: Minimally Invasive and Cost-Effective

The case focused on evaluating the patient's dental-facial esthetics and tooth size discrepancies. With ongoing orthodontics, planning options like digital smile design and injection molding were discussed. The goal was a balanced, esthetic result with minimal invasiveness and cost, ultimately favoring injection molding as the most efficient solution.

Where do we put the teeth within the face?

In this complex case, the patient has implants, veneers, and crowns, with issues of gray discoloration and slight lip imbalance from previous surgeries. Dental Facial Analysis (DFA) revealed excessive tooth exposure. The plan is to shorten the anterior teeth, possibly use Invisalign to intrude the canines, and select suitable materials. Splinting the implants with pink porcelain to achieve a natural papilla effect is recommended. Effective lab communication is crucial for optimal material selection and esthetic outcomes.

Harmony in Esthetics: Choosing the Right Materials

During this session, we’ll explore the intricacies of finding the balance between esthetic goal and minimizing the risks associated with tooth structure integrity, which play a crucial role in material selection for achieving a beautiful outcome. Special guest Nelson Rego, CDT, AAACD will elaborate on the considerations and decision-making process involved in selecting the most appropriate material for this particular patient case.
